What Does a GCM Do?
I step in when you need help coordinating and securing services to insure you are getting the care you need to maintain the quality of life you want. This may be a one-time meeting to clarify your current situation and plan for changes, or it may mean an on-going management of care if you or a loved one are no longer wanting to deal with all the details.
I charge by the hour. Most insurance plans don’t cover these costs, and Medicare does not pay for this service. Depending on needs, you can expect to pay anywhere from $1,000 to $4,000 per month.
Services include, but are not limited to:
- Make home visits and suggest needed services
- Evaluate in-home care needs
- Coordinate medical services
- Recommend and assist in selecting care personnel
- Assist you in making short- and long-term plans
- Facilitate conversations around difficult topics & complex issues
- Address emotional concerns
- Evaluate alternative living arrangements
- Provide caregiver stress relief

While the bulk of coordination takes place Monday through Friday during normal business hours, I also make myself available as needed on weekends and evenings in the event of an emergency or unplanned change.
Coordination includes recommendation of providers, screening of caregivers/ agencies, and scheduling.
